Who We Are
We are one of three project sites funded by DSHS in Washington State to
improve the health of children in foster care. We are located at the
Child and Adolescent Clinic in Longview, WA and ensure coordinated care
for children in Foster Care. Staff include the medical director, program
director, counselor, and medical assistants.

Vision Statement
We are a center that supports clinics to provide comprehensive and coordinated
medical care for children in foster care using the American Academy of
Pediatrics tenets of the medical home which results in improved health
outcomes for participating children. Foster care includes all children
who are currently in an out-of-home placement, including kinship care.

Hours of Operation
Program Staff are available during regular business hours; patients of
the clinic can access their primary care clinician at 360-577-1771 during
business hours or 360-577-1200 after hours with urgent health concerns.

Goals
-
Create a
written medical home care plan for each child that is shared with
the DSHS social worker and pediatrician and is the basis for
coordinated care.

Ensure
mental health screening is provided to all children.

Promote
engagement in the Progress Center’s Tracking Learning Children
(TLC)program (for those children less than 5 years of age).

Improve
rates of children receiving a timely IHE (Initial health
examination) – a required evaluation within 72 hours of removal from
home.

Improve
rates of children receiving the 30-day EPSDT: required well child
exam within 30 days of removal from home.

Empower and
support foster parents in the management of each child’s medical
issues and improve their knowledge of the management of chronic
health conditions by scheduling a visit with the pediatrician with
each change in foster care placement and as needed.

Message to Foster Parents
The
Child and Adolescent Clinic is here to support you and ensure that every
child receives comprehensive coordinated care. Please contact us with
any medical questions or concerns, and you are welcome to schedule a
parent conference with the pediatrician or nurse practitioner.
We strongly encourage you to schedule a clinic appointment within three
days of a child being placed with you, even if the child is not new to
the foster care system. This can benefit you and the child by increasing
your understanding the child’s medical history, reviewing current
medications, ensuring prescriptions are up-to-date, giving you additional
information and support, and discussing how to best manage chronic health
problems and keep the child healthy through this transition.
