Scheduling a Meet and Greet is a great way for prospective parents and new patients to ask questions, see how we work, and meet one of our providers. We want to help you make the best decision for your family.

Transferring Medical Records
When you call to schedule an appointment with our practice, we will ask you to bring a copy of your child's immunization records.
Once you arrive we will obtain a Release of Information (ROI) to collect additional medical records from your previous providers office.
Our Goal Is To Provide The Best Medical Care
To do this successfully, we must be able to review all of your medical records from all providers that you have seen. It is our policy to receive your previous medical records before we can assume care and schedule an appointment.
